Preputial adhesiolysis (also referred to as foreskin adhesion release) is a procedure that separates the foreskin from the glans penis when the two remain adherent. In most cases, this simple procedure is performed during childhood.

A foreskin adherent to the glans is completely normal in newborn boys, and in most cases the adhesions resolve spontaneously during the first months or years of life. However, if the foreskin cannot be retracted over the glans at a later age, it may lead to problems with hygiene and, eventually, sexual function.

In addition to persistent foreskin adhesions, inability to retract the foreskin may also be caused by phimosis (tightening of the foreskin).

How preputial adhesiolysis is performed

The procedure is preceded by an initial examination to determine whether the inability to retract the foreskin is caused by adhesions or by phimosis.

If adhesions are present, they are treated by adhesiolysis. If phimosis is diagnosed, circumcision is the recommended treatment.

Preputial adhesiolysis is performed on an outpatient basis under local anaesthesia. During the procedure, the adhesions are gently released using a fine probe while the foreskin is gradually retracted towards the base of the penis. Once the adhesions have been separated, an antibiotic ointment is applied to the glans and the inner layer of the foreskin.

The procedure is very quick, usually taking no more than 10 minutes, and you can return home immediately afterwards.

Recovery after preputial adhesiolysis

It is normal to experience mild discomfort or slight pain in the treated area for a short period after the procedure.

For several days afterwards, the glans and foreskin should be treated with the prescribed ointment. Until complete healing, the foreskin should also be gently retracted towards the base of the penis every day to prevent the adhesions from reforming.

Gentle washing of the penis with lukewarm chamomile or agrimony herbal infusion is also recommended, as these herbs have soothing and antiseptic properties.
